Summary:
Valproic acid may initially cause a decrease in total phenytoin
level by displacement of phenytoin from protein binding sites
(Prod Info Depakote(R) ER, 2003; Levy & Koch, 1982; Bruni et al, 1980;
Monks et al, 1978).
This reflects a decrease in the bound fraction of phenytoin; the
phenytoin which is displaced by valproic acid re-equilibrates with the
large capacity of the tissue compartment such that the unbound plasma
concentration remains unchanged (Winter, 1988; Mattson et al, 1978).
The degree of displacement appears to be valproic acid dose related
(Monks & Richens, 1980).
Valproic acid may also inhibit phenytoin metabolism
(Levy & Koch, 1982; Bruni et al, 1980; Patel et al, 1980; Winter, 1988;
de Wolff et al, 1982).
VPA會置換PHT在albumin的binding site，
使得binding form減少，free form增加。
但是這些被擠出Albumin的游離態PHT跟組織室的親和力很高，
所以又會binding到其他組織室的蛋白上去，
因此大抵而言free-form concentration是不會有太大變化的。
所以整體來看，Total form是會下降一些些的。
另外，在臨床上，關於PHT的TDM：
Neonates: 8-15 mcg/mL for total form ; 1-2 mcg/mL for free form
Adults: 10-20 mcg/mL for total form ; 1-2.5 mcg/mL for free form
監測Total form: 當病患Albumin正常，無其他疾病或藥物使用。
監測Free form: 1. 當病患在治療濃度範圍內產生副作用時。
2. 慢性腎衰竭。
3. 肝臟疾病。
4. 使用與Albumin有高度結合力的藥物。
所以我想泡麵大應該疑惑的應該是 total form 的點吧~
另外，因為VPA會抑制CYP2C9，所以長期使用的話是有可能會抑制PHT的代謝。

2F：→ pharmawind:應該是指total，因為臨床上似乎不會測binding，而是用 03/24 00:47
3F：→ pharmawind:換算的；但不管看total還是binding，PHT的確都是下降的 03/24 00:47
