標題[問題] nz申請表格要怎麼寫?

明天一早就要開放申請了
不過光表格第一個問題就答不出來..
不知道大家第一個問題都怎麼回答??
a1.which working holiday scheme are you applying for a visa/permit under?
另外第三個部份的聯絡人
c2.name and address for communication about this application.
可以只填寫自已嗎??
最後一個部份的

請問是要勾選那個部份??
是 i do not have ...這一個嗎??
