标题[问题] nz申请表格要怎麽写?

明天一早就要开放申请了
不过光表格第一个问题就答不出来..
不知道大家第一个问题都怎麽回答??
a1.which working holiday scheme are you applying for a visa/permit under?
另外第三个部份的联络人
c2.name and address for communication about this application.
可以只填写自已吗??
