標題[問題] DV 260 填表疑問請教

我在5月時已經將DS260送出去了
但今天再檢查一次時發現了幾個錯誤.....
Provide information on your last five U.S. visits.
我只填了一個 必須要填滿5個嗎？
Do you have documentation to establish that you have received vaccinations in
accordance with U.S. law?
我是選yes 但現在想想 是哪些疫苗是美國法律規定要打的啊... ？
Are you a former exchange visitor (J) who has not yet fulfilled the two-year f
oreign residence requirement?
這個我是選yes. 但我是大學的J1- WORK AND TRAVEL 這個算嗎？
還有一欄是
Have you attended any educational institutions at a secondary level or above?
我當時選了No 現在看來是必須要選擇yes
那我現在該做的事 是不是要寫email去KCC unlock 我的ds260?
