In the presence of distant metastases, the clinical sensitivity
of CA 15.3 in different studies has ranged from
50% to 90% depending on the anatomical site (e.g. liver
metastases are associated with the highest sensitivity,
followed by skeletal and lung metastases) and the number
or size of metastatic foci. Combining the results from
seven different studies, it was shown by the Expert Panel
of ASCO that about two-thirds of patients (235 out of
352) had CA 15.3 elevations either before or at the time
of recurrence, with a lead time ranging from 2 to 9
months [25, 44, 45, 46, 47, 48, 49, 50]. In the same
meta-analysis, 1,320 patients out of 1,435 without evidence
of recurrence had normal CA 15.3 levels (diagnostic
specificity of 92%).
大意是說: CA 15-3在偵測遠端轉移的敏感度約50-90%
(針對不同的部位有不同的敏感度, 肝轉移最高, 骨和肺次之)
ASCO的報告, 有2/3的病人在發現復發之前, 或者發現復發當時
有CA 15-3上升的情形 約可提前2-9個月出現
至於專一度, 約有92%
敏感度的定義: 實際上有復發的病人中, CA 15-3升高的比例
專一性的定義: 實際上沒有復發的病人中, CA 15-3的值正常的比例
專一性高 代表偽陽性低 也就是說 CA 15-3上升時
很可能的確有復發了
如果一般檢查找不到問題
家中也沒有經濟壓力的話 可以考慮自費做正子造影(PET)
或者可以和主治醫師商量有沒有辦法幫你開
(健保局真的會刪PET! 所以背景不夠硬的醫院, 開健保給付的PET是自殺的行為 >.<)
當然, 提早找到位置 不代表可以做更好的治療, 或會有更佳的預後
