刚刚用google查了一下 rule out的定义: (diagnosis term) A working diagnosis. Your healthcare provider thinks this is the most likely diagnosis, and will either confirm or discard it (rule it out), based on the results of further tests or investigations. A working diagnosis is a diagnosis that is known to be uncertain, but which is used to define a course of action. Note that the working diagnosis is not necessarily the most likely item in the differential. 举例而言:(一个国外blog: http://trots.blogspot.com/) 在2005/2/23的文章有写到关於differential diagnosis "一个病人来急诊抱怨胸痛" 当时的DD可能是:pneumonia, heart attack, pulmonary embolism.....等等 经过interview,PE...(对不起,我省略很多) 医生觉得pneumonia最有可能 但又排除不掉Pulmonary embolism 可能会排个lung scan... 此时working diagnosis就是"R/O PE"(即使pneumonia是心里第一顺位) (当然若是能写更详细是更好) 若是结果是equivocal的或是low probility... 此时的诊断变成"pneumonia, R/O PE" PE还是有可能,不过顺位就没有那麽前面了 Rule out带有着点negative finding的意涵.... ---------------------------------------------------------------------- 比较明确的结论是:R/O A病可用於住院时的诊断。 不过住院诊断工作 (work up) 之後，许多诊断应该是已经被ruled in 或ruled out。 如诊断仍未被确认，而A病还是最有可能 则写成Discharge diagnosis: probable A病 或suspected A病较为适当。 感觉rule out~~要满小心的写 P.S.: 1.野人献曝,整理的有点乱....:) 2.不过关於报case时要怎麽用...还是不太明白 Tentative diagnosis要放在那一张好呢?? 毕竟每个r/o,感觉後面要有对应的work-up Sorry...当完兵..都忘了 --
