标题[文法] 不定词的被动语态与过去式

想请教各位版友如何去判定不定词的被动语态与过去式！
因为在Azar文法书看到的一题例题，让我不知如何判别，
心里也有些疑惑，如下：
The witness to the murder asked not (identify) _____
in the newspaper.She wanted her name kept secret.
空格的答案为：to be identified
我想请教的是因为已有asked出现，所以此句要用 to be identified吗?
另外，为何此句不能使用to have been identified此过去被动式？
问题很蠢，还请见谅。
