标题[求译] 这段想了很久还是翻不通顺，恳求帮我改一下，感谢！

我的试译：
1.此宪章为走向新的公共卫生设立了挑战，重申社会的公平与正义，
是健康的首要条件，并将倡导及调解视为实现这些目标的过程。
提问：
1.我觉得这句子的动词是set,受词应该有challenge、advocacy及mediation，
请问这样的想法对吗？
或者是最後两个and的意思是:
by reaffirming..., and by advocacy, and by mediation...?
2.我想请问"as the processes for their achievement"这一小段
是修饰mediation还是修饰这一小段之前的整个句子？
我这段想了好久好久，还是觉得都不通顺，
拜托各位前辈帮我改正一下，感激不尽！
