※ [本文轉錄自 Gossiping 看板] 作者: mk (壞公投法是藍營通過的!!!) 看板: Gossiping 標題: [爆卦] 健保局:台灣健保美國稱羨 時間: Wed Jan 23 20:14:49 2008 專業論壇討論文章： 　http://doctorvoice.org/viewtopic.php?f=1&t=11690 原文出處： 　http://www.nhi.gov.tw/webdata/webdata.asp?menu=1&menu_id=&webdata_id=2145 還是得去找原文期刊看才不會被健保局呼嚨... === 最新一期的英國醫學期刊(BMJ|12 Jan 2008|volume 336)在其「觀測站專欄」中刊登美 國知名醫療經濟學者Uwe Reinhardt的文章「台灣健保美國自嘆弗如」，作者主要表達台 灣健保行政效率高，美國比之相形遜色，因此美國應可從台灣經驗中得到啟發。 作者Uwe Reinhardt教授任教於美國普林斯頓大學，專長是政治經濟、經濟與公共事務， 亦是我國行政院科技顧問。Reinhardt教授過去擔任過美國科學院醫學會院士、美國國 家顧問團健康照護政策顧問及美國衛生部顧問，在2002年現代健康照護期刊的讀者票選 他為美國健康政策最具影響力的前十大名人。 在這篇文章中，Reinhardt教授認為台灣健保背後有強而有力的資訊系統做為支撐，可以 有效管理醫療費用的申報，未來更有可能先美國一步推動電子病歷系統，以增加管理效 率。同時由台灣經驗可知，總額支付制度是短期上有效的醫療費用控管工具，但長期而 言，仍需有具彈性的成本控制方法，以為因應。 另外，在醫療品質方面，Reinhardt教授認為由於台灣與美國整體投入醫療的資金比例不 同，不應將台灣的醫療品質與美國頂級的醫療品質相比，而是應該以健保開辦前後的醫療 品質作比較，並且台灣如果能再增加投入1~2%的GDP於醫療服務上，台灣的醫療系統會有 大幅的改善，亦會降低民眾就醫時的自付費用。同時健保局過低的行政經費也應該再提高 ，藉由台灣強而有力的資訊平台，台灣會比美國更容易提升效率及品質。 最後Reinhardt教授也稱許台灣健保的開辦使得台灣的病患不致因各人經濟能力不同， 而得到上、中、下，三級式的差別待遇，因此即使台灣的勞工受到隣近國家低價勞力市 場的衝擊，他們仍享有全民健保這面安全網的保障，健保，確實為國家珍貴的資產。 原文網址及摘要如下： http://www.bmj.com/cgi/content/full/336/7635/72 BMJ 2008;336:72 (12 January), doi:10.1136/bmj.39450.473380.0F Observations Atlantic Crossing Humbled in Taiwan Uwe E Reinhardt, James Madison professor of political economy, Princeton University, Princeton, New Jersey [email protected] Taiwan’s highly efficient system of national health insurance should humble and inspire the United States The first 150 words of the full text of this article appear below. Tagging along with Tsung-Mei Cheng, an expert on Taiwan’s health system, on her recent visit to Taiwan’s Bureau of National Health Insurance, turned out to be a bit humbling for me as someone who focuses mainly on the US health system. The bureau is the government agency that administers Taiwan’s single payer national health insurance system. Its staff members fret when hospitals and walk-in clinics fail to submit completed claims within the required 24 hours after delivery of service. Private health insurance companies in the United States count themselves lucky if high priced actuaries can tell them in the middle of the year what the carrier ultimately will have to pay the providers of health care for services rendered in the previous year.
