我想加入一句類似「保證所填寫內容一切屬實」之類的句子(翻成英文)
（也許不只這句但我對這種文件沒什麼概念>"<）
請幫我翻譯這一句，如果您對這種文件有其他建議或需要加入的字句也請不吝加入
另外，「授權人簽名」和「被授權人簽名」該怎麼翻？謝謝！
