Extracts of household registration records may be obtained by furnishing the nam e of the person, date of birth, names of parents, and name of spouse, if married . Care should be taken to ensure that the applicant is submitting a properly certified extract of the household registration, and not just a photo copy of his Household Roster card. A properly certified extract will have a stat ement stamped on it, and will be impressed with the seal of the household regist ration in red ink.
