标题[问题] 关於狂犬病疫苗的要求

小的最近准备申请去北卡的兽医教学医院做observer
所需要准备的申请表格其中一个是rabieswaiver
表格需要填的内容如下:

我现在有两个问题
1.我一定要去打狂犬病疫苗吗?虽然表格上看起来可以拒绝施打，
但是这样会不会申请比较不会过?
2.最後一项破伤风的疫苗，我四年前有打过，不过我该怎麽申请证明呢?
是去原本施打的医疗机构申请还是怎样?
一望有经验的前辈可以提供一些经验
