糟糕有点晚看到这篇（已经寄了QQ） 大哈上没有B12缺乏出现神经症状的比例 只说"Cobalamin deficiency may cause a bilateral peripheral neuropathy" （这边最好附上网路上找的资料） 而在诊断的地方写说 "The diagnosis of cobalamin or folate deficiency has traditionally depended on the recognition of the relevant abnormalities in the peripheral blood and analysis of the blood levels of the vitamins." 所以应要分析血液方能确诊，而非端看有无神经症状 後面folate deficiency治疗的地方又说 "Before large doses of folic acid are given, cobalamin deficiency must be excluded and, if present, corrected, otherwise cobalamin neuropathy may develop." 所以没有确诊应不适合单独给folate 也就是如你所说应该是B+C 建议BC皆可 我等下有事没空去寄，有人还没寄可以帮忙一下吗？ ※ 引述《slhoivnee (Mr.青春)》之铭言： : 病患为巨母红血球性贫血(megaloblastic anemia) 可能情况有缺乏B12与Folate : 虽然题目有说明不具神经性异常~理应为Folate型 : 不过我查这个网站 : http://www.aafp.org/afp/990315ap/1598.html : 在提到Vitamin B12 的第一段倒数第四行那里~ : Complete or partial resolution of neurologic symptoms occurs in as many as : 80 percent of patients. : 有约80%的人会出现神经症状 : 换句话说五个人里面可能有一个人是没神经症状症状型的Vitamin B12缺乏 : 另外在同一个网站提到Folate的最後一段提到 : However, because the use of folic acid supplementation partially corrects the : hematologic abnormalities of vitamin B12 deficiency but not the associated : neurologic deterioration, other experts have recommended that all foods : fortified with folic acid also include vitamin B12 supplementation. : 若只提供叶酸~虽然B12型的症状会改善~但无法治疗神经损伤 因此专家们都建议提供 : folate的的同时一并给予B12以防万一~ : 我质疑的点是~虽然神经症状是B12型的典型症状~但是在没有其它资料的辅助佐证下 : (比方说过去病史，个人史，甚至是性别年龄)似乎难以准确判断是否真是folate缺乏 : 因此最适当的疗法为(B)+(C) : 不过我手边没有教科书可以查~感觉这一题有机会... : 可以请板上的大家帮个忙提供一下资料吗?(或者是帮寄也可以) : 建议是(B) (C)皆给分 : 感谢大家 --
