以下内容供没有上到课的同学读讲义抓重点参考 主要是学长上课提到(以及我有抄到:p)比较重要的东西 正确性不敢保证 有些重点也可能有遗漏 有错请尽量提出 #DMS <gross motor> 2个月会抬头, 四个月会翻身(记得把床围栏拉起来!!) 七坐八爬 一岁会走路，一岁半上楼梯，两岁下楼梯，两岁半双脚跳，三岁骑三轮车，3岁半单脚跳 <fine motor> 三个月会自己把手打开 六个月时在小朋友脸上盖卫生纸，他会伸手把指拿开（为一个"cross midline"的动作） 三圆四方五三角(三岁画圆四岁画圆五岁画三角形) <social skill> 1y/o: 叫他他会来 2y/o: play with others <language> 1岁会讲一两个像"爸爸","妈妈"之类的单词 2会的时候会说"主词加动词"的句子 #NE 1.头围：前六个月一个月长一公分，6~12m/o一个月长0.5cm 3.前囟门18个月时关闭，後囟门3.4个月时就已经关闭 4.hydrocephalus sign Maceween sign:敲头的声音听起来像敲水壶 transillumination:用蛇灯一照看起来像灯笼一样 5.Facial dysmorphism要怀疑genetic problem 6.有Torticollis要用echo确认有没有SCM hypertrophy，如果没有要怀疑可能是brain的问 题 <skin>可能是配合题!! cafe au lait spot: neurofibromatosis hypopigmentation:tuberus sclerosis (更正) neurofibroma:neurofibromatosis facial nevus:Sturge-Weber syndrome telangiectasia:ataxia-telangiectasia skin lesions over lumbosacral area(hemangioma, pit, hair): meningocele <primitive reflex> Moro reflex(0~6m/o)不对称时要怀疑brachial plexus injury ---------------Seizure and epilepsy 基本观念：每一次seizure attack对brain都是伤害，抽久了brain会产生新的放电点，每 次看到seizure都要治疗，不然会造成intractable seizure #Fever with seizure 要记得"meningitis 非死即伤" #Febrile convulsion 1.6m/o~6y/o，小於一岁的小朋友要很注意他们在sizure之後的活力，因为他们的 meningeal sign不明显，要很积极地考虑作lumbar puncture 2.simple febrile seizure的特徵 (1)BT>39oC (2)generalized tonic clonic seizure (3)secs~10mins (4)common with viral infection (5)F.H.(+) 3.complex seizure以後变成epilepsy的机会比较高，特徵如下 (1)focal seizure (2)>15 mins (3)repeated episodes with the same illness (4)low grade fever #status epilepticus 1.generalized tonic clonic seizure is life threatening，会造成CO2 retension, shoking 2.最常见的原因是 a febrile seizure in an otherwise normal child #Infantile spasm 1.seizure pattern是点头、像要抱住东西的样子 2.attack通常是在快睡着或是要醒来的时候 3.EEG finding: hypsarrhythmia #Cerebral Palsy 1."static" encephalopathy 2.clinical point: epilepsy, GI disorder(需要PEG,长期便秘), infection (aspirarion pneumonia) --
