标题[问题] DV中签填表问题?

DS-230 表格第35个问题:
List dates of all previous visits to or residence in
the United States. (If never, write "never")
Give type of visa status, if known.
Give DHS "A" number if any.
实在是看不懂最後一句话"Give DHS "A" number if any."
有网友可以帮忙解答吗?谢谢!
另外,DSP-122表格上有写道:
Using the enclosed self-adhesive return address label, immediately
send this form along with DS-230 to...
查了前人的经历,DV2010之前中签者会收到通知package,
内有标签贴纸,
DV2012中签者是自行上网查询得知
那...这self-adhesive return address label要怎麽附上阿?
